County of Santa Clara Primary Care Access Program (PCAP)

The Primary Care Access Program (PCAP) is a locally funded program that provides primary care services to residually uninsured residents in Santa Clara County through designated community clinics and health centers. On November 3, 2015, the Santa Clara County Board of Supervisors voted unanimously to approve the PCAP.

PCAP is available to individuals meeting the following criteria:

  • Age 19 or older
  • Resident of Santa Clara County
  • Family Income less than 200% of the Federal Poverty Level
  • Not eligible for:
    • Full Scope Medi-Cal coverage
    • Purchase of insurance through the Covered California exchange
    • Employer provided health insurance
